Currently, we accept cash, credit cards, debit cards, and checks. You may save and update your credit card information at any time through our Ensora portal login.

Referrals

Parents & Caregivers

If you'd like to begin therapy services, contact your primary care physician and request that a referral be sent to our office via fax at (808) 856-0115.

You must specify if you'd like to receive occupational or speech therapy services. Sometimes, the physician will ask what the concern is. At this point, you may state your concerns and why you believe speech or occupational therapy can help.

Once we receive the referral, we will reach out to you to begin the initial client intake.

  • Therapy discipline (Speech and/or Occupational Therapy)

  • Ordering physician's name and NPI

  • Facility (Maui Speech and Swallow)

  • Primary diagnosis

  • Physician's most recent clinical notes

Frequently Asked Questions

  • This depends entirely on your insurance company and the type of insurance plan you have. Services must be medically necessary for insurance to cover therapy sessions. The clinician will write a report using the test results from your first appointment or evaluation. This report is submitted to the insurance company to obtain approval for therapy. In general, HMSA PPO/HMO and University Health Alliance do not cover speech therapy treatment for children. However, this too depends on what type of insurance plan or line you may have with HMSA PPO/HMO and UHA.

  • Yes, we can! We have private pay options and we offer a sliding scale fee for those parents who qualify. Please contact the front office for more information regarding the sliding scale fee.

  • Therapy treatment sessions are $75 for 25 minutes, $112.50 for 40 minutes, and $150 for 50 minutes.

    Evaluation sessions are usually covered by most insurance companies. Please inquire about private pay options for evaluations by calling (808) 856-9821.

  • Yes. Most insurance companies require a physician's referral to begin therapy services.

  • Most of our sessions take place at our clinic. We have a hybrid program for our voice patients where sessions are conducted via telehealth in your home. Sometimes, sessions can also take place via telehealth if you cannot make it to our clinic. We currently have a waitlist for in-home therapy sessions. If you'd like in-home sessions, you may request to be placed on our waiting list for the next therapist to provide that in your home.

  • We request that all cancellations be made at least 24 hours before your appointment, out of courtesy. However, we understand that emergencies happen. You may text, call, or leave a voicemail at (808) 856-9821 to cancel your appointment. All cancellations must be rescheduled on a different date and time than your regularly scheduled appointment.

    If you refuse to reschedule your appointment, you may be charged a $30 fee (If you cancelled 24 hours in advance), a $50 fee (If you cancelled the day of or no-showed), or you may be placed on a waitlist or on a non-recurring schedule depending on your attendance.

    If you plan to be gone for an extended period of time, you may keep your recurring time slot by rescheduling those appointments before or after your vacation. If you do not want to reschedule your appointments, you will be taken off the schedule and re-established with a new date and time for recurring therapy times slots.

    If there are excessive cancellations, no-shows, or requests for rescheduling, you may be taken off of our recurring schedule.

  • We understand that emergencies can happen. All cancelled sessions will be made up at a different date and time. For example, if you normally have therapy on Wednesdays at 1:00 pm, we would have you come in on a Tuesday to make up for the previous session, and you would come again the next day on Wednesday at 1:00 pm, your regularly scheduled date and time.

  • Yes. You can stop therapy services at any time. Please email the office at admin@mauispeechandswallow.com to immediately stop services in writing.

  • Yes, we offer occupational therapy for children, teens, and adults; however, we only offer occupational therapy services for neurological impairments at this time.
